Travel Vaccines
Prior to your Travel appointment you will need to complete an assessment form which you can find below.
Please print the form, complete it and bring it to the surgery. You will then be given an appointment with the nurse.
Most Travel Vaccines are provided by the surgery although for some specialised vaccines patients will be referred to an appropriate specialised centre. A charge may be payable for some travel vaccines. Further details are available from the practice.
Please ensure that you arrange your Travel Consultation as soon as you know you are travelling. Some vaccines require more than one dose to be effective and some must be given several weeks before travelling.
